I sometimes forget to perform sajdah sahw - Do I have to repeat my namaaz?

In the Name of Allah, the Most Gracious, the Most Merciful. 

As-salāmu ‘alaykum wa-rahmatullāhi wa-barakātuh. 

There are two possibilities to your query,

1. One remembers he made a mistake after the prayer time has expired
2. One remembers he made a mistake within the prayer time
   
    
   
   If one remembers he made a mistake after the prayer time has expired, in the inquired situation the Salaah should not be repeated.^[i][i]
   
    
   
   However, if one remembers he made a mistake within the prayer time then the following must be considered.
   
   If after completing Salaah,

1. One carried out an action which nullifies the Salaah (e.g. Turning your chest away from the qiblah once having left the masjid) then the Salaah will be nullified and should be performed again.
2. One has not carried out an action which nullifies the Salaah, then perform Sajdah Sahw and complete the Salaah as usual, by reciting Tashahhud, Durood and then Salaam.^[ii][ii]

(بَابُ سُجُودِ السَّهْوِ) (قَوْلُهُ يَسْجُدُ لِلسَّهْوِ) مُقَيَّدٌ بِمَا إذَا كَانَ الْوَقْتُ صَالِحًا حَتَّى أَنَّ مَنْ عَلَيْهِ السَّهْوُ فِي صَلَاةِ الصُّبْحِ إذَا لَمْ يَسْجُدْ حَتَّى طَلَعَتْ الشَّمْسُ بَعْدَ السَّلَامِ الْأَوَّلِ سَقَطَ عَنْهُ السُّجُودُ، وَكَذَا إذَا سَهَا فِي قَضَاءِ الْفَائِتَةِ فَلَمْ يَسْجُدْ حَتَّى احْمَرَّتْ، وَكَذَا فِي الْجُمُعَةِ إذَا خَرَجَ وَقْتُهَا، وَكُلُّ مَا يَمْنَعُ الْبِنَاءَ إذَا وُجِدَ بَعْدَ السَّلَامِ يُسْقِطُ السَّهْوَ.

وَيَأْتِي بِالصَّلَاةِ عَلَى النَّبِيِّ - عَلَيْهِ الصَّلَاةُ وَالسَّلَامُ -، وَالدُّعَاءِ فِي قَعْدَةِ السَّهْوِ هُوَ الصَّحِيحُ لِأَنَّ الدُّعَاءَ مَوْضِعُهُ آخِرُ الصَّلَاةِ.

(ويسجد للسهو ولو مع سلامه ) ناويا ( للقطع ) لأن نية تغيير المشروع لغو ( ما لم يتحول عن القبلة أو يتكلم ) لبطلان التحريمة ، ولو نسي السهو أو سجدة صلبية أو تلاوية يلزمه ذلك ما دام في المسجد .

وَلَوْ سَلَّمَ وَعَلَيْهِ تِلَاوِيَّةٌ وَسَهْوِيَّةٌ غَيْرَ ذَاكِرٍ لَهُمَا أَوْ ذَاكِرًا لِلسَّهْوِ خَاصَّةً لَا يُعَدُّ سَلَامُهُ قَاطِعًا، فَإِذَا تَذَكَّرَ يَسْجُدُ لِلتِّلَاوَةِ أَوَّلًا ثُمَّ يَتَشَهَّدُ وَيُسَلِّمُ ثُمَّ لِمَا قَدَّمْنَا مِنْ أَنَّ سَجْدَةَ التِّلَاوَةِ تَرْفَعُ الْقَعْدَةَ ثُمَّ يَسْجُدُ لِلسَّهْوِ وَيَتَشَهَّدُ وَيُسَلِّمُ

اعلم أن الوجوب مقيد بما إذا كان الوقت صالحا حتى إن من عليه السهو في صلاة الصبح إذا لم يسجد حتى طلعت الشمس بعد السلام الأول سقط عنه السجود
