Crohn’s Disease - Do i have to fast?
Question
As Salam Alikum Wa Rahmatulahe Wa Baraka Tu
My question is based on a medical condition and fasting.
The name is called Crohn's disease. This is a condition where your body immune system attacks itself sending out white blood cells causing inflammation in my intestine leading to ulceration, bleeding and frequent diarrhea. Eventually, this constant inflammation will lead to scar tissue building up causing the intestine to narrow. This is the case with me. This condition causes other complications.
I'm currently having 3-7 bowel movements a day, losing blood, urgency for the toilet, losing weight and undigested food passing straight through me. I suffer from mild - moderate pain too. I slip into depression frequently and mainly because of my condition and looking at my two very young children as I don’t have the strength to give them my best as a father should.
This condition is different for everyone. It can attack any part of the digestive system and different levels of severity. I have been fasting this Ramadhan and have done so in the past with this condition being active for 9 years. I'm currently awaiting to see a surgeon for planning of an operation as surgeons believe this is something I will benefit from. I am on various medication but i have now become lazy taking. Should I continue to fast during Ramadhan? If not, what is the best thing to do?
I have in the past not fasted on days i have been weak. But this is rare. Somedays i am fine for a good few hours and then suddenly my symptoms become very active. For example, yesterday during the day i was fine until i started praying Taraweeh and i need an urgent bowel movement which resulted in an accident. To me this is become the norm and I don’t know what normality is anymore.
Fasting since the age of 10 has enabled me to experience the beauty of Ramadhan that ALLAH has bestowed on us and not fasting makes me feel guilty and left out but at the same time i don't want to be in a position where it is Haram as the body has a right over me.

Answer
In the name of Allah, Most Gracious, Most Merciful
As-salaamu ‘alaykum wa-rahmatullaahi wa-barakaatuh
We take note of the difficulty you are in and make Dua that Allah Ta’ala make it easy for you. Aamīn.
You state you have the Crohn disease. You have been fasting during Ramadān and have done so in the past with this condition being active for 9 years.
The Crohn’s disease can cause serious complications such as abnormal connections between the digestive system and other parts of the body, including weight loss, fever and feeling of a mass or fullness in the abdomen.[1]
We advise you to consult with a competent Muslim doctor to see if you have a genuine excuse not to fast.[2]
If the doctor says that fasting will not be detrimental to your health, then fasting will be compulsory.
However, if he advises that you shouldn’t fast, then you will be excused from fasting. In this case, you will give fidya (compensation) for not fasting.[3]
The fidyah for each fast is similar to how much one gives for sadqah al-fitr.[4]That is to give 1.6 kg of wheat or its value to the poor as fidyah for each missed fast.[5] However, if you recover from your sickness and you are able to fast again, you are required to make up all the fasts you missed, even if fidayah was already paid for the missed fasts.[6]
You may recite the following verse abundantly and blow on yourself:
أَنِّي مَسَّنِيَ الضُّرُّ وَأَنتَ أَرْحَمُ الرَّاحِمِينَ (سورة الأنبياء: 28)
Difficulty have afflicted me, and you are the most merciful. (Surah al-Ambiya: 83)
Note- When the doctor advises that you shouldn’t fast, you will be excused from fasting only during the period the Crohn’s disease is active. It is possible the Crohn’s disease may be active during one Ramadān and not the following Ramadān.
May Allah Ta’ala give you complete Shifā. Aamīn.

ونحن نقول إن زيادة المرض وامتداده قد يفضي إلى الهلاك فيجب الاحتراز عنه وطريق معرفته الاجتهاد فإذا غلب على ظنه أفطر وكذا إذا أخبره طبيب مسلم حاذق عدل والصحيح الذي يخشى أن يمرض بالصوم فهو كالمريض وكذا الأمة التي تخدم إذا خافت الضعف جاز أن تفطر ثم تقضي

(لمن خاف زيادة المرض الفطر) لقوله تعالى * (فمن كان منكم مريضا أو على سفر فعدة من أيام أخر) * (البقرة: 184) فإنه أباح الفطر مريض لكن القطع بأن شرعية الفطر فيه إنما هو لدفع الحرج وتحقق الربح منوط بزيادة المرض، أو إبطاء البرء أو إفساد عضو، ثم معرفة ذلك باجتهاد المريض والاجتهاد غير مجرد الوهم بل هو غلبة الظن عن أمارة أو تجربة أو بإخبار طبيب مسلم غير ظاهر الفسق

(قوله خاف الزيادة) أو إبطاء البرء أو فساد عضو بحر أو وجع العين أو جراحة أو صداعا أو غيره، ومثله ما إذا كان يمرض المرضى قهستاني ط أي بأن يعولهم ويلزم من
صومه ضياعهم وهلاكهم لضعفه عن القيام بهم إذا صام (قوله وصحيح خاف المرض) أي بغلبة الظن كما يأتي، فما في شرح المجمع من أنه لا يفطر محمول على أن المراد بالخوف مجرد الوهم كما في البحر والشرنبلالي

ونقل المصنف في حاشية الدر عن الزيلعي من عوارض الصوم ما نصه الصحيح الذي يخشى أن يمرض بالصوم فهو كالمريض اهـ قال فكذلك هنا
أما المريض فالمرخص منه هو الذي يخاف أن يزداد بالصوم واليه وقعت الإشارة في الجامع الصغير ، فإنه قال : في رجل خاف إن لم يفطر تزداد عيناه وجعاً أوحماه شدة أفطر ، وذكر الكرخي في مختصره : أن المريض الذي يبيح الإفطار هو ما يخاف منه الموت أو زيادة العلة كائناً ما كانت العلة . ( بدائع الصنائع : ۲ / ۹۶ ، سعيد ، فصل في حكم فساد الصوم )

والصحيح الذي يخشى أن يمرض بالصوم فهو كالمريض، ومراده بالخشية غلبة الظن كما أراد المصنف بالخوف إياها. وأطلق الخوف ابن الملك في شرح الجمع وأراد الوهم حيث قال: لو خاف من المرض لا يفطر. وفي فتح القدير: الامة إذا ضعفت عن العمل وخشيت الهلاك بالصوم جاز لها الفطر،

( ويجوز الفطر لشيخ فان وعجوز فانية ) سمي فانيا لأنه قرب إلى الفناء أو فنية قوته وعجز عن الأداء ( وتلزمهما الفدية ) وكذا من عجز عن نذر الأبد لا لغيرهم من ذوي الأعذار ( لكل يوم نصف صاع من بر ) أو قيمته بشرط دوام عجز الفاني
