Please advise the source (Hadith) of Du’aa-e-Mehfil (the one you recite at the end of a gathering)?

Also kindly correct the transliteration and provide the translation of this Du?aa: Subhanallahi wa Bihamdihi, Subhanak Allahumma wa Bihamdika, Nash hadu al-Laahilaaha Illa Anta Nastaghfiruka Wa-anatub Ilaik.
